Flexion or Extension: The Movement Pattern You May Be Missing

Pattern recognition is a big part of what I do every day. Over the years, I’ve noticed that people often lack either flexion or extension throughout their bodies.

You may even recognize this in yourself. Some movements or exercises may feel natural and comfortable, while others consistently feel restricted, awkward, or harder. These tendencies can offer useful information about how your body moves and which positions are harder for you to access.

If you divide the body into front and back, many front-side muscles are associated with flexion. In contrast, many of the muscles on the back side are associated with extension. Of course, there are exceptions, but this gives us a simple way to start thinking about flexion and extension patterns throughout the body.

When you think of flexion, think of movements such as touching your toes or bringing your chin toward your chest. Extension is the opposite: think of a backbend, reaching your arm straight behind you, or lifting your chin as if you were looking up at something.

The shoulder is a little different. Reaching your arm overhead is considered shoulder flexion, even though the arm is moving upward rather than folding the body forward. Someone who lacks flexion may struggle with movements that require the body to fold or close down, while someone who lacks extension may struggle with movements that require the body to open or move backward.

What makes this interesting is that these patterns often show up in more than one area of the body.

Looking at the Pattern, Not Just the Joint

These patterns show up in movements people struggle to regain every day. Have you ever worked with—or experienced this yourself—someone who lacks hip extension, thoracic extension, and ankle dorsiflexion?

Although dorsiflexion is the anatomical term for this motion at the ankle, I look at what is happening at the foot as part of the larger movement pattern. As the lower leg moves forward over the foot, the foot and ankle must move and adapt to allow that position. When that motion is limited alongside restrictions in hip and thoracic extension, I start to look beyond each joint and consider whether they are connected as part of a larger pattern.

If you look globally rather than locally, you may begin to recognize this as an extension issue. If you zoom in and look only at the local restriction, then, in my opinion, you’re missing an important part of the picture.

For example, someone who lacks hip extension or ankle dorsiflexion may focus entirely on the local tissues to improve that specific movement. They may stretch the hip flexors, mobilize the ankle, or repeatedly work on the restricted joint. While these strategies may improve motion locally, they don’t necessarily address why the same pattern is showing up in multiple areas of the body.

When I see several of these restrictions occurring together, I don’t immediately view them as separate joint problems. I start asking what could be influencing the larger pattern and why the body is having difficulty accessing extension in the first place.

What About Flexion?

The same thing can happen in the opposite direction. Some people don’t struggle primarily with extension—they struggle to access flexion. When you assess different areas of the body, you may begin to see the same pattern repeated from one area to another.

For example, someone may always have tight hamstrings. They stretch their hamstrings, feel better for a minute or two, and then the tightness returns. As I mentioned earlier, touching your toes is a flexion pattern, and to achieve this position without compensation, much of the body needs to flex.

If you can’t touch your toes, it’s easy to assume that your hamstrings are the problem. But what if the hamstrings are only one part of a larger flexion restriction?

Stretching them may temporarily change how they feel without addressing why your body has difficulty accessing flexion in the first place.

Movement Is More Than Muscles

So, what could cause someone to have difficulty accessing flexion or extension throughout the body?

This is where the nervous system becomes important.

Flexion and extension aren’t simply determined by whether a muscle is tight or weak. The brain and nervous system help regulate muscle tone, posture, and the coordinated activity required to produce movement. Several areas of the nervous system contribute to these responses.

Within the brainstem, the midbrain, pons, and medulla contain pathways and centers that influence posture and muscle tone. Some of these pathways can have different influences on flexor and extensor activity. The vestibular system and cerebellum also play important roles in regulating postural tone, balance, and our ability to organize movement against gravity.

The visual system can influence these responses as well. Your eyes don’t function separately from the rest of your body. Eye position and visual input interact with brainstem, vestibular, and postural systems. Changing where you look can influence head and body positioning and alter muscular activity elsewhere in the body.

One example involves neurological connections within the pons associated with eye movement, including the abducens nerve, which controls the lateral rectus muscle and moves the eye outward. Rather than thinking of one nerve as directly creating extension throughout the body, I look at these structures as pieces of a much larger network connecting eye movement, the brainstem, vestibular function, posture, and movement.

This is why I don’t automatically view a tight or restricted muscle as the cause of a movement problem. If multiple areas of the body are showing a similar pattern, I want to understand what may be influencing that pattern rather than simply treating each muscle or joint individually.

Why the Pattern Matters

None of this means that stretching, mobility work, or strengthening a specific area is useless. Sometimes a local restriction really does need local attention.

The problem is assuming that the location of the restriction automatically tells you its cause.

If your hip won’t extend, stretching your hip flexors seems logical. If your ankle doesn’t move well, mobilizing your ankle makes sense. If your hamstrings always feel tight, stretching them seems like the obvious answer.

But what happens when you repeatedly work on those areas and the same restriction keeps returning?

That’s when I think it’s worth stepping back and looking at the entire person.

Is the same directional restriction appearing elsewhere? Does changing something away from the restricted area immediately change the movement? Are you dealing with several unrelated mobility problems, or are those problems different expressions of the same underlying pattern?

Those questions can change how you approach the problem.

The Body Leaves Clues

A restriction isn’t always the problem itself. Sometimes it’s information.

The more areas you assess, the clearer the pattern can become. One restricted movement may tell you very little. The same type of restriction appearing repeatedly throughout the body tells you much more.

This is why pattern recognition is such an important part of how I assess movement. I’m not simply looking for the tightest muscle or the joint with the least range of motion. I’m trying to understand how the pieces fit together.

If you have spent months or years stretching the same muscle, mobilizing the same joint, or working on the same movement without lasting change, it may be time to ask a different question.

Instead of asking, “What’s tight?”

Ask, “Why does my body keep returning to this pattern?”

That question may bring you much closer to the root cause.

Instead of repeatedly treating the area that feels tight or restricted, step back and look at the entire pattern. Sometimes the location where you feel the problem is only one piece of the puzzle.
